To ensure smooth and timely claim settlement, participants are required to submit complete and accurate documentation as per claim type.

1. After Claim Intimation – Required Documents
  • Death Claim Forms
  • Original Policy Membership / Certificate Documents (PMD)
  • Account Maintaining Certificate issued by Bank
  • Discharge Voucher with IBAN Number
  • Family Registration Certificate (FRC)
  • Hospital & Lab Test Reports & Doctor Prescriptions Record (if any)
  • First Information Report (FIR) in case of Accidental Death (if any)
  • Post Mortem / Autopsy / Medico Legal Report (if any)
  • Newspaper Report of incident (if any)
2. Accidental Death – Required Documents
  • Hospital & Lab Test Reports & Doctor Prescriptions Record (if any)
  • First Information Report (FIR) in case of Accidental Death (if any)
  • Dead Body Snaps / RESCUEE Report (if any)
  • Post Mortem / Autopsy / Medico Legal Report

Yes, authentic and proper Account Details are required to disburse the payment to nominee
• Succession Certificate of NADRA/Court of Law required in case of death of both the Participant & Nominee
• Guardianship Certificate of concern Court of Law required in case of death of Guardian
• Succession Certificate simply means a Court Declaration/Order/Decision wherein honorable Court decide how much share could be paid to remaining legal heirs of deceased
• Guardianship Certificate simply means a Court Declaration/Order/Decision that who will be the next Guardian/Custodian of minor nominee after the death of his/her Parents/Guardian appointed before
In case of loss of original PMD, a declaration required on E-Stamp Paper given by company representative
